Case Evaluation Quiz

What we collect

If someone completes some or all of the quiz, we collect: their name, email address, and phone
number (if provided); their answers about the accident (type of accident, fault, injuries,
medical treatment, impact on work, timing, state, and whether they already have an attorney);
the case score the quiz calculates from those answers; and basic technical data (IP address and
browser user-agent string) recorded at the time of submission. If someone starts the quiz but
doesn’t finish it, their in-progress answers are also saved as they go, so we can follow up if
they’d like to pick up where they left off.

• Accident details: type, fault, injuries, treatment, work impact, timing, state, attorney status
• Case score: calculated from quiz answers
• Technical data: IP address, browser user-agent
• Partial submissions: in-progress answers saved (for follow-up)

Why we collect it

We use this information to evaluate whether smith Jakson may be able to
help with a potential case, and to contact the person who submitted it about next steps. We do
not sell this information, and we do not share it with other law firms or lead-generation
networks.

How long we keep it

If the quiz is started but never finished, that partial data is kept for up to
30 days and then permanently deleted.
For completed submissions, we keep the full record — including name, email, and phone number —
for up to 730 days (about 24 months).
After that, we remove the personal contact details from the record but keep the non-identifying
case information (such as the case score and general case type) for our own internal reporting.
